La facciata testimonia di per sé la complessa vicenda edilizia del complesso del Duomo, con la sedimentazione di secoli di architettura e scultura italiana.Cinque campiture fanno intuire la presenza della navate, con sei contrafforti (doppi alle estremità e attorno al portale centrale) sormontati da guglie. La costruzione della facciata cominciò nel 1590, sotto la direzione dell'architetto Pellegrini, in stile tardomanierista, continuando poi nella prima metà del Seicento sotto la direzioni del Richini e di Carlo Buzzi. Risalgono a quel periodo i cinque portali e parte delle finestre soprastanti, con il coronamento a timpano spezzato. La decorazione a bassorilievo dei portali venne scolpita ai tempi dell'arcivescovo Federico Borromeo su disegni del Cerano. I basamenti dei contrafforti centrali sono decorati da rilievi seicenteschi, con telamoni disegnati da Carlo Buzzi. I rilievi sui basamenti dei contrafforti laterali sono invece del XVIII e XIX secolo. A partire dalla metà del Seicento infatti lavori procedettero a rilento a causa dell'acceso dibattito sulla scelta del progetto da adottare.
